Candidates are to report for verification of documents and Interview on 10.07.2014 at Conference Hall (1st Floor) of Jawahar Lal Nehru Hospital and Research Centre (JLNH & RC), Main Hospital Sector – 9, Bhilai, C.G. at 09.00 a.m. along with following documents in original (for verification only) with one set of self-attested photocopy and two colour passport size recent photograph (as submitted during online application in website) attested by Gazetted Officer:-
1. Marks Sheet of Matriculation indicating Date of Birth of the candidates
2. Certificate and Mark sheets of all years/Semester of Degree in Pharmacy from a recognised institution
OR
Certificate and Mark sheets of all years/Semester of (1) Diploma in Pharmacy of minimum 2 years duration from a recognised Institution (2) 10+2 3. Certificate of valid registration in Indian/ State Pharmacy Council
4. One year post qualification experience certificate in Hospital/Nursing Home as on 17.03.2014.
5. SC/ST/OBC Certificate from a Revenue Officer not below the rank of Tehsildar in Central Government format for appointment to posts under the Government of India and Central Government Public Sector Undertaking (Format available on our website www.sail.co.in).
6. NOC from the employer in case the candidates are presently employed in Government Department/PSU/Autonomous bodies.
7. Photo Identity Card (viz. Driving License, PAN Card, Voter ID Card, Aadhar Card etc.)
Candidates should note that, they must produce the above mentioned documents in original otherwise they will not be allowed to appear in the interview.
Outstation ST/PwD candidates will be reimbursed single to and fro rail fair (2nd class sleeper) by shortest route on production of proof of journey perform. Therefore, candidates must bring proof of journey performed to attend this interview at Bhilai from their address as submitted at the time of online application.
Candidates are advised to report for Interview as per schedule date, time and venue. No separate call letter (in hard copy) will be issued.
Candidates must bring the below given form (Details of the Candidate) completely filled up, signed by them with their photograph affixed.
